World is progressing very fast in dealing with various problems related to Mankind. Surgery has taken a paradigm shift to Minimal Invasive Procedure and concepts of many problems are changing with better management and result.
Modern Information Technology has made World as a family with knowledge and newer developments are reaching to everyone on a mouse click.
High research in medicine in basic science leading to change in epidemiology and causative factors from environment, genetic to metabolic genome theory in Malignancy, may develop a different treatment Module.
Science is to research and rediscover facts since centuries with changing concepts. It will go on. But basic problems faced by large population and surgeons in particular remains same. Common causes of death are Trauma (Motor Vehicle Accidents), Cardio Vascular Disorder (Blocking of Vessel), Inflammatory Disorder and Malignancies.
80% of our Surgeons in Asian Countries and 60% of Surgeons in Western Countries are involved in treating common problems as above successfully with modern management. It needs disciplined undergraduate and post graduate curriculum and training. Every Country desires its structure suitable to them. After undergraduate (small %) and post graduate (higher 30%) students understands and has a desire to learn more and better. Seek admission in other countries or in Higher Centers (limited seats) as per availability and siyuation. U.K. is the prime place to get fellowship (post graduate degree) and USA for better training under its Residency Programme .Small number to other European Countries and Australia.
WHY SO ?
From 1960’s to 2000 a phase of freedom, development in under developed and developing countries in basic needs of education, health, infrastructure, food etc. Few countries have developed more in there per Capita Income and mean average of standard of leaving.
Since 2000 developing countries including ASEAN and SAARC countries have take a leap forward to improve their leaving index and health care system. Two parts – (a) Health to the people at three levels – from peripheral distant village and town to State Cities – giving basic care in all fields of Medicine Covering 80% of problem (b) High Health Care Hospitals including Medical College Hospitals and Corporate Hospitals.
Gradual improvement at both levels with better care.
Many Corporate and Government National Institution are will developed and compete with any World Class Institution anywhere in the world. Last 20 years have seen a dramatic improvement in high Medical Centre and quality care with Research this part of the World.
Medical Education is also under going drastic change for good.
In India more than 450 Medical College are there with more than 50% in private sectors providing more than 65000 undergraduates and 25000 post graduates in various specialities. Similar is the situation proportionately in other Asian Countries also.
On an average 2000 Doctors leave country for better opportunities or getting better training to develop Skill. USA gets the most followed by U.K. and Australia – Everyone Happy. Now a large number of Doctors are gradually coming back with good training, and helping in developing Institutions all over the country. This is a sea change in last 15 years. Medical tourism is increasing with high % every year from neighbouring countries to Middle East and Europe also.
Similarly other Asian Countries have developed very well in Health field providing all kinds of high tech procedure.
This is to continue with high pace, but under countries political system. These countries have learnt a lesson to improve its Health System.
Systems of admission to medical courses have changed the mind set of students from practical training to theoretical learning. Learning even basic operative procedures is after post graduate degrees either here or abroad.
Many good Centres including National Institutions are giving good primary and higher teaching and training but to a small percentage. All Medical and Surgical specialities and sub specialities are organizing many Workshops, based on training and CME’s to update knowledge and refine surgical techniques. More than 500 such activities are undertaken by ASI, its Chapters and Speciality Sections, and doing a good job. Still much is desired to cover peripheral Surgeons.
Similarly many good academic activities are going on in other Asian Countries, but limited to its own areas.
With economic progress more money is spent in development of Health Care System. It need Coordination to know about such good activities going on in each Asian Countries and well planned calendar of events to be known to all Surgeons/Medical personal.
Every Country and every Surgeon has to offer something worthwhile to learn. We all have good potential and teach and learn to equip our Surgeons to do quality work and improve Surgical Care. It needs all of us to work in Cooperation and make our platform with identified centers initially with designated subjects of training. Surgeons from these countries as well Resource persons can join to form a curriculum and calendar for learning and teaching. Surgeons from one country can go in other Asian Country and take training in subject of his choice; similarly Resource person can deliver their Skill and Knowledge to help other centres. A free flow of students and teachers from one country to other to learn and teach from each other is the need of time.
Western Countries system is well developed as per there requirements and is a very well formed Module to train Surgeons and help people in general. There experts can help us in developing Asian System and form a World Forum for Health Care and Prosperity to better living.
You all are Surgeons of high Skill and Wisdom, Leading your Surgical Association and Know all aspects in field of Surgery from technical, administrative to political system. What better talent to talk and discuss to evolve a way and proceed gradually.
World Congress of Surgery is celebrating its 46th Conference at Bangkok (Thailand) from 23rd to 27th August 2015. Let us take one step ahead and resolve to act together, design basic structure and curriculum and start acting at WCS in Bangkok.
I am optimist and great hope of cooperation among all participating countries; gradually involve other countries to spread knowledge for bettermentof the humanity.
